You have a Word document with survey or quiz questions. You need them in Microsoft Forms. Unlike Google Forms, Microsoft Forms actually accepts Word document uploads directly via its Quick Import feature. The challenge is getting the import to go cleanly, and knowing what to do when flagged items appear. This guide covers all three methods and explains exactly what happens at each step.
Three Methods: Which One Is Right for You?
| Method | Time (50 questions) | File Types | Start Review Flags |
|---|---|---|---|
| Manual copy-paste | 3-4 hours | Any | None (manual control) |
| Native Quick Import | Under 5 minutes | Word, PDF only | Varies by document structure |
| Pirai AI (recommended) | Under 15 minutes | Word, PDF, Excel, CSV, TXT, XML, QSF | Minimal: output is structured for clean import |
Method 1: Manual Copy-Paste
This works for any file type and requires no external tools. For large surveys it is impractical.
-
1Go to forms.office.com and create a new form Sign in with your Microsoft account at forms.office.com and click New Form.
-
2Add each question manually from your Word document Click Add new for each question. Select the question type, paste the question text, and enter each answer option by hand.
-
3Repeat for every question, then share For a 50-question survey this takes 3-4 hours. For 100 questions, plan for 6-8 hours. When done, click Send to share.
When to use it: You have fewer than 10 questions and no external tools. For anything larger, Method 2 or 3 is significantly faster.
Method 2: Native Quick Import (Word and PDF only)
Microsoft Forms has a built-in Quick Import feature that accepts Word documents and PDFs directly. It is the fastest method when your document is already well-structured. The catch is that complex formatting, embedded objects, and unsupported question types will appear as flagged items in Start Review.
-
1Go to forms.office.com and select Quick Import Sign in at forms.office.com. On the home page, select Quick Import.
-
2Upload from this device Choose Upload from this device. Select your Word (.docx) or PDF file. Choose Form or Quiz depending on your use case.
-
3Wait for conversion and review Microsoft Forms converts the file and autosaves it under Recent and My Forms. Select Start Review to check the results and resolve any flagged items.
Understanding Start Review
After Quick Import, Microsoft Forms flags any content it could not convert cleanly. Three types of feedback appear: Missing content (content that was skipped entirely), Uncertain conversion (content Microsoft Forms is unsure how to handle), and Unsupported question type (types not supported on import such as matrix grids). Select each flagged item, make the correction, and click Resolved. The more structured your source document, the fewer flags appear.
Limitations: Quick Import only supports Word and PDF. It does not accept Excel, CSV, TXT, or XML. It is not available for US Government Cloud accounts. File size limit is 10MB.
Method 3: Pirai AI, Any File Type, Fewer Flags
Pirai AI converts any document format into a .docx specifically structured for Microsoft Forms Quick Import. The output is formatted to minimise flagged items during Start Review and handles question type adaptation automatically.
| Your File | Extension | Common Source |
|---|---|---|
| Word document | .docx | Microsoft Word, Google Docs (downloaded) |
| Native PDFs, scanned documents, Word exports | ||
| Excel spreadsheet | .xlsx | Survey specs, question banks in tabular form |
| CSV file | .csv | Structured question lists |
| Plain text | .txt | Simple questionnaire text |
| XML file | .xml | LimeSurvey, SurveyMonkey exports |
| Qualtrics survey | .qsf | Migrating from Qualtrics to Microsoft Forms |
You can also paste plain text directly into the platform without uploading a file.

How Question Types Are Converted
Microsoft Forms natively imports Multiple Choice and Open Text. Pirai AI handles all other types by adapting them to the closest supported format rather than skipping them.
| Source Question Type | Converts To | Notes |
|---|---|---|
| Multiple choice (single answer) | Multiple Choice | Native, imports cleanly |
| Open text / short answer | Text (short answer) | Native, imports cleanly |
| Open text / long answer | Text (long answer) | Native, imports cleanly |
| Multiple choice (multi-select) | Single choice | Adapted (Microsoft Forms limitation) |
| Matrix / grid questions | Individual single-choice questions | Adapted: one question per statement row |
| Form / multi-field text | Individual text questions | Adapted: one text question per field |
| Rank order | Multiple choice | Adapted: options listed as choices |
| Constant sum | Text question | Adapted: respondent enters values as text |
| Descriptive text (DB) | Not imported | Skipped: not supported by Microsoft Forms |
| Page breaks | Not imported | Skipped: Microsoft Forms has no page structure |
After import: Any adapted question type can be manually changed in the Microsoft Forms editor. Pirai AI preserves the question content so you only need to adjust the type, not re-enter the text and options.
Time Comparison: Three Methods
| Survey Size | Manual Copy-Paste | Native Quick Import | Pirai AI |
|---|---|---|---|
| 10 questions | 30-45 minutes | Under 5 minutes | Under 10 minutes |
| 50 questions | 3-4 hours | Under 5 minutes | Under 15 minutes |
| 100 questions (mixed types) | 6-8 hours | 5-10 minutes + Start Review | Under 15 minutes, fewer flags |
| Excel / CSV input | Manual only | Not supported | Under 15 minutes |
