How to Convert a Word Document to Microsoft Forms

You have a Word document with survey or quiz questions. You need them in Microsoft Forms. Unlike Google Forms, Microsoft Forms actually accepts Word document uploads directly via its Quick Import feature. The challenge is getting the import to go cleanly, and knowing what to do when flagged items appear. This guide covers all three methods and explains exactly what happens at each step.

Three Methods: Which One Is Right for You?

MethodTime (50 questions)File TypesStart Review Flags
Manual copy-paste 3-4 hours Any None (manual control)
Native Quick Import Under 5 minutes Word, PDF only Varies by document structure
Pirai AI (recommended) Under 15 minutes Word, PDF, Excel, CSV, TXT, XML, QSF Minimal: output is structured for clean import

Method 1: Manual Copy-Paste

This works for any file type and requires no external tools. For large surveys it is impractical.

When to use it: You have fewer than 10 questions and no external tools. For anything larger, Method 2 or 3 is significantly faster.

Method 2: Native Quick Import (Word and PDF only)

Microsoft Forms has a built-in Quick Import feature that accepts Word documents and PDFs directly. It is the fastest method when your document is already well-structured. The catch is that complex formatting, embedded objects, and unsupported question types will appear as flagged items in Start Review.

Understanding Start Review

After Quick Import, Microsoft Forms flags any content it could not convert cleanly. Three types of feedback appear: Missing content (content that was skipped entirely), Uncertain conversion (content Microsoft Forms is unsure how to handle), and Unsupported question type (types not supported on import such as matrix grids). Select each flagged item, make the correction, and click Resolved. The more structured your source document, the fewer flags appear.

Limitations: Quick Import only supports Word and PDF. It does not accept Excel, CSV, TXT, or XML. It is not available for US Government Cloud accounts. File size limit is 10MB.

Method 3: Pirai AI, Any File Type, Fewer Flags

Pirai AI converts any document format into a .docx specifically structured for Microsoft Forms Quick Import. The output is formatted to minimise flagged items during Start Review and handles question type adaptation automatically.

How to Convert with Pirai AI: Step by Step

How Question Types Are Converted

Microsoft Forms natively imports Multiple Choice and Open Text. Pirai AI handles all other types by adapting them to the closest supported format rather than skipping them.

Source Question TypeConverts ToNotes
Multiple choice (single answer)Multiple ChoiceNative, imports cleanly
Open text / short answerText (short answer)Native, imports cleanly
Open text / long answerText (long answer)Native, imports cleanly
Multiple choice (multi-select)Single choiceAdapted (Microsoft Forms limitation)
Matrix / grid questionsIndividual single-choice questionsAdapted: one question per statement row
Form / multi-field textIndividual text questionsAdapted: one text question per field
Rank orderMultiple choiceAdapted: options listed as choices
Constant sumText questionAdapted: respondent enters values as text
Descriptive text (DB)Not importedSkipped: not supported by Microsoft Forms
Page breaksNot importedSkipped: Microsoft Forms has no page structure

After import: Any adapted question type can be manually changed in the Microsoft Forms editor. Pirai AI preserves the question content so you only need to adjust the type, not re-enter the text and options.
